Meloxicam, the main active ingredient of Mebilax, is a new generation NSAID belonging to the oxicam class, has anti-inflammatory, analgesic and antipyretic properties. A common mechanism for the previously mentioned effects is due to meloxicam has ability to inhibit the biosynthesis of prostaglandins, the known mediators of inflammation, pain and fever.
Meloxicam is well absorbed after oral administration, with an average bioavailability of 89%. The drug strongly combines with plasma proteins, mainly with albumin. Meloxicam is strongly metabolized at liver, about a half of the substance is excreted in urine and the remain in the stool. The mean elimination half-life of meloxicam is 20 hours.
Meloxicam has high potency in the treatment of osteoarthritis, is well tolerated especially in the gastrointestinal tract. These features of meloxicam makes patients highly satisfied when using it.
Symptomatic treatment of chronic pains in:
- Painful osteoarthritis (arthrosis, degenerative joint disease).
- Rheumatoid arthritis.
- Ankylosing spondylitis.
Allergy to any components of the drug.
Mebilax should not be given to patients who are allergic to aspirin and other NSAIDs.
Progressive gastro-duodenal ulcer. Severe hepatic impairment. Nondialysed severe renal impairment.
Children under 12 years. Pregnant women and breast-feeding mothers.
Cautions should be exercised in patients with a history of upper gastrointestinal disease and in patients receiving treatment with anticoagulants. Mebilax should be withdrawn if gastro-duodenal ulceration or gastrointestinal bleeding occurs.
The dose of meloxicam in patients with end-stage renal failure should not exceed 7.5 mg per day. No dose reduction is required in patients with mild or moderate renal impairment. Mebilax should be stoped and follow-up tests should be carried out in case of elevations of serum transaminases or other parameters of liver functions have been reported.
The concomitant use of Mebilax and other NSAIDs increases the risk of gastrointestinal ulceration and bleeding. The concomitant administration of Mebilax and anticoagulants, thrombolytics increases risk of bleeding.
Meloxicam increases lithium plasma levels, hematologic toxicity of methotrexate, and nephrotoxicity of cyclosporin. The concurrent use of meloxicam and diuretics increases the potential for accute renal insufficiency in patients who are dehydrated.
Meloxicam reduces the effects of antihypertensive drugs.
Gastrointestinal disturbances e.g diarrhea, stomachache, dyspepsia, nausea, flatulence. Dizziness, headache, tinnitus. Itching, skin rash, urticaria.
The cases of anemia, disturbances of blood count, ulceration or gastrointestinal bleeding rarely occur.
Inform your physician about any adverse effects occur during the treatment.
In case of overdose, the standard measures for first-aid should be carried out. There is no known specific antidote at present. It has been shown in a clinical trial that cholestyramine accelerates the elimination of meloxicam.
Severe injuries on the gastrointestinal tract can be treated by antacid drugs or H2-antihistamine agents.
DOSAGE & ADMINISTRATION
Rheumatoid arthritis, ankylosing spondylitis: oral dose of 2 tablets daily. According to the therapeutic response, the dose may be reduced to 1 tablet daily.
Painful osteoarthritis: oral dose of 1 tablet daily. If necessary, the dose may be increased to 2 tablets daily.
In patients with increased risks of adverse reactions, start treatment at the dose of 1 tablet daily.
In dialysis patients with severe renal failure, the dose should not exceed 1 tablet daily.
